Organizations can mail these forms to ma.aemaplans@gov.ab.ca with the subject line "CESW Bulk Staff Form". when complete, or may choose to retain this information in readiness for a possible announcement from government that prioritized supports are available. Your information will be stored in a secure location with need-to-know access to protect personal information.

This form allows organizations to list Tier 1, 2 and 3 workers:

The definition of Tier 1 workers is:

  • High-Value, Low-Density Experts
  • Occupy a single point of failure in a complex system.
  • eg. industrial control / process experts (e.g. control room supervisor).

Tier 2 – Limited pool of specific expertise

  • Specialist doctors, nurses, others with specialized training in healthcare
  • Key government / EM officials

Tier 3 – Large pool of “fungible” employees with redundant skills

  • Police, fire, and EMS; corrections and social services staff
  • Select industrial tradespeople and transportation workers
